We get ours from J. J. Keller 800-327-6868, They make these books(Vehicle Inspection Report Detailed Duplicate Copy Book) with the carbon copy paper that our guys keep in the truck and fill out each day. Not sure about the price, we've been using them so long.

JJKeller is expensive; but they are the very best.

They have a service: Keller online that has the answer to every DOT or OSHA question that you could ever ask.

ours are triplicate and have all ca dot stuff and we put our own things on there as well for the pumps. ie, reducers hyd hoses and so on.

We use , computer logs     paper logs    fuel tickets   vehicle inspection sheets.Our company made the vehicle inspection form,paper logs were bought at truck stop,a years worth for 9 dollars.
