I believe that this happened due to putting the added weight of the beam at the same place as oputrigger.

The breach in the ground was seen by the spot man but chose to continue the pick.The old we can do it caper,drop load move crane to safety,then continue with pick.I'm glad this came back around,a great lesson learned here.
